HEALTH INSURANCE REIMBURSEMENT PROCEDURES

Beginning December 1, 2018

Proof of payment documents for the retiree’s medical insurance reimbursement can be submitted from December 2018 thru February 2019. Checks may take 60 days to process.
Acceptable Documents:
Documentation must support the calendar year period of time for which you are seeking payment. Acceptable forms of proof of coverage and payment are:
Premium notices or Insurance Provider Statements and canceled checks
Spouse’s employer coverage statements (or letters) showing proof of coverage and premium paid for the Retiree’s coverage only
Medicare Parts A and/or B statements (i.e. Form SSA-1099)
Social Security statements showing Medicare premium deductions paid
SURS annuity payment statements showing medical premium deductions (CIP or TRAIL premiums)
